Effective January 1, 2026, Select Survey will be retired. All users currently have access to begin converting their existing forms to the new platform in advance of this date. As of January 1, 2026, new form creation within Select Survey will no longer be available. Existing forms and reporting access will remain active for a limited period following retirement to allow for transition and data access.
The replacement solution is
Microsoft Forms. Please note that Protected Health Information (PHI) is not permitted to be collected using Microsoft Forms. Any questions about this transition or forms that require PHI must be submitted through a request to
Digital Services for secure form development.
